GLP-1 medication
Two molecules do most of the work in this field, and they are sold under four well-known brand names plus a tablet. Knowing which is which makes the rest of the decision much easier, and it is the part most people arrive confused about.
Two molecules
Semaglutide is the older of the two and mimics one gut hormone, GLP-1. It is sold as Ozempic, which is approved for type 2 diabetes, as Wegovy, which is approved for weight management, and as a daily tablet.
Tirzepatide mimics two hormones, GLP-1 and GIP. It is sold as Mounjaro for type 2 diabetes and as Zepbound for weight management. In head-to-head trial data it produced more weight loss than semaglutide, which is why the comparison page exists.
Which one you are prescribed depends on your history, what you tolerate, what is actually available, and what your drug plan will fund. All of them are prescription-only and all of them are decided by a clinician, not chosen from a menu. Related reading: Semaglutide, Tirzepatide and Semaglutide vs tirzepatide.
Getting a prescription
BMI, conditions, personal and family history, and everything else you take. Some histories rule a GLP-1 out entirely, and that is decided here.
Semaglutide or tirzepatide, injectable or tablet. Availability and your drug plan are part of the decision, not an afterthought.
Every GLP-1 starts low. The starting dose is not a treatment dose, it is there to let your body adjust.
The dose steps up on a schedule, and only if the current step is tolerated. Rushing this is what causes most of the misery people describe.

Brands versus molecules, injections versus tablets, and how the choice is made.
I had the brand names muddled completely. Ten minutes of explanation and I finally understood what I was actually being prescribed and why.
Switched molecules after the first one did not agree with me. That was treated as a normal adjustment rather than a failure.
The tablet suited me better than a weekly injection. The instructions about timing and food were stricter than I expected but they were explained clearly.
They checked what my cardiologist had me on before writing anything. Two things needed spacing out.
