BMI thresholds
Two numbers do most of the work: 30, and 27. Above 30 you generally meet the threshold on weight alone. Between 27 and 30 you meet it when a weight-related condition is documented alongside.
The numbers
BMI is weight divided by height squared. At 30 and above it is generally accepted as the threshold for weight-management medication on weight alone.
Between 27 and 30 the threshold applies when a weight-related condition is documented. That documentation matters: a suspicion is not the same as a diagnosis, and insurers in particular want it on record.

Working it out
Height without shoes, weight first thing. Small errors in height affect the result more than people expect, because it is squared.
Weight in kilograms divided by height in metres squared. Any calculator will do it, and the number is the same everywhere.
If you land between 27 and 30, whether you have a documented weight-related condition is the deciding factor.
A clinician checks the figure against your records, because self-reported height and weight drift.
Its limits
BMI does not distinguish muscle from fat, so it overstates risk in people carrying a lot of lean mass and understates it in people carrying little. It also says nothing about where fat sits, which matters more than the total for metabolic risk.
