It exists so you can find out whether a consultation is worth paying for before you pay for one. Two minutes, no card, and the answers are read by a clinician rather than scored by a form.
What it asks
Height and weight for the BMI. Then the conditions you have been diagnosed with, because those decide whether the 27 threshold applies to you rather than 30.
Then everything you currently take, including anything prescribed by another specialist and anything over the counter. This is the part people leave incomplete and it is the part that most often changes the answer.
Finally the screening questions: family thyroid history, past pancreatitis or gallbladder trouble, and whether pregnancy is possible or planned. Do I qualify, Who should not take it and How it works go further into it.

Rough numbers are fine at this stage. Completeness on the medication list matters more than precision on the weight.
Before anyone calls you. That means the first conversation starts from your history rather than from scratch.
Either a consultation is worth booking, or it is not, or something specific needs checking first.
Nothing is booked automatically and nothing is charged. The assessment is information, not a commitment.
What it is not
It cannot diagnose you and it does not produce a prescription. Anyone offering medication off the back of an online form alone is skipping the part that makes treatment safe.
What it does is save you the cost of a consultation you were never going to benefit from, and give the clinician something to work from when you do have one. More on that in How it works, Book a consultation and Program cost.
